We are seeking a full-time, salaried Medically Complex Population Staff to promote and protect the health and well-being of the Medicaid Eligible Individuals (EIs) of the general care program.
Must strive to assure that Eligible Individuals (EIs) receive comprehensive and cost-effective health care with primary focus on children who are medically fragile with congenital/acquired multi-system diseases.
The schedule is Monday-Friday (8am-5pm).
Candidates can reside in one of the six (6) counties we serve:
Cullman, Jackson, Limestone, Madison, Marshall, and Morgan.
Qualifications/
Experience:
Minimum Licensed Master Social Worker (LMSW) with experience in a pediatric environment.
Preferred experience settings include acute hospital, intensive care, Children's Rehabilitation, Children's Specialty Clinic, Children's Mental Health, or pediatric clinic.
Current Alabama Driver's License and Auto Insurance Reliable Transporation
Essential Functions:
Conduct psychosocial assessments to identify clients' needs and develop a care plan when there is a need for complex High or Medium-Risk care management.
Implement medication reconciliation with the PCP and Network Pharmacist to assure continuation of needed therapy on all EIs stratified as High / Medium-Risk and on all transition of care EIs.
Identify and address barriers (internally and externally) that prevent EIs from securing appropriate care in the most cost-effective manner.
Address issues pertaining to NACC care goals to enhance access, insure quality and control costs.
